Over the last few days, I've been creating a short survey for our directorates safety day booth (DLI) which is going to be held on 5/22 at Presidio of Monterey. The purpose of the survey is to ask booth visitors whether or not they found the information gathered at the booth on disaster preparedness helpful/useful. The other constructs gauge their level of preparedness and ask them to check items or steps they plan to take.
